St. Faustina died in 1938.

She wrote that Jesus taught her to pray those words.

HOLY GOD

HOLY IMMORTAL ONE
HAVE MERCY ON US

However, as I posted, those words have been part of the Church's liturgy since before the year 900. So the words are more than 1,107 years old.

The question would have been whether a young Polish nun with virtually no education could have possibly made the connection.

St. Faustina would have heard it every year at the Good Friday Liturgy.

Indeed. The Trisagion has been around for an eternity. It is used liberally in the Byzantine Rite.
